Objective To explore the situation of drug use in hospitalized children, and to provide scientific data for the improvement of management system. Methods 400 cases of pediatric patients admitted to the Department of Pediatrics from January 2015 to April 2016 were selected. With reference to the China Food and Drug Administration approved latest version of the drug manual, the off-la-bel usage in medication cases and medical advices were determined, and the drug type, age distribution and department distribution of these off-label usages were observed. Results There were 3960 medical prescriptions in 400 cases of hospitalized children, in which 1622 prescriptions in 379 cases had off-label usage, including overdose ( 30. 15%) , unspecified medication information ( 29. 41%) , over-indication ( 20. 04% ) , over-age ( 14. 67% ) , over-dosing times ( 5. 06% ) , over-administration route ( 0. 68% ) . Infants and children were the highest proportion of off-label usage, with the top 3 departments being the Department of General Pediatrics ( 23. 18%) , Respiratory Nephrology ( 21. 27% ) and Neonatology ( 20. 90% ) . Conclusion The situation of children inpatients′ off-label medication is not opti-mistic, which indicates that the clinicians are not regulated in pediatric medication. The supervision and management should be strength-end to standardize the situation to ensure the safety of pediatric medication.%目的 探讨住院患儿超药品说明书用药情况,并分析原因,为完善管理制度提供科学资料.方法 随机抽取2015年1月至2016年4月医院儿科住院部收治的400例患儿,参照国家食品药品监督管理总局批准的最新版药品说明书对超药品说明书用药病例及医嘱进行判定,观察超药品说明书用药类型、年龄分布及科室分布.结果 400例住院患儿共涉及医嘱3960条,有379例共1622条医嘱存在超药品说明书用药情况,超药品说明书用药类型包括超量(30.15%)、未标注儿童用药信息(29.41%)、超适应证(20.04%)、超年龄(14.67%)、超给药次数(5.06%)、超给药途径(0.68%);婴幼儿和儿童是超药品说明书用药比例最高的2个年龄段;超药品说明书用药患儿数量及医嘱数量居前3位的科室分别为普儿科(23.18%)、呼吸内科(21.27%)及新生儿科(20.90%).结论 住院患儿超药品说明书用药形势不容乐观,临床医师在儿科用药方面存在较多不规范,需要加强监督管理,以规范超药品说明书用药,从而保证儿科用药安全.
