急性冠状动脉综合征患者冠状动脉病变程度与血清基质金属蛋白酶9表达的相关性分析

摘要

目的 探讨急性冠状动脉综合征(ACS)患者冠状动脉病变程度与血清基质金属蛋白酶9(M MP-9)表达的相关性.方法 选择2011年8月到2013年12月在湖北医药学院附属东风医院进行诊治的ACS患者80例作为观察组(不稳定型心绞痛45例,急性心肌梗死35例),选择同期无ACS的80例患者作为对照组,对2组临床一般资料、血液生化与血清MMP-9表达进行回顾性分析.结果 观察组血清MMP-9含量明显高于对照组[(125±14) μg/L比(87±14) μg/L,P＜0.05],同时急性心肌梗死患者的血清MMP-9含量明显高于不稳定型心绞痛患者[(146±14) μg/L比(111±14) μg/L,P＜0.05].Logistic回归分析发现MMP-9比值比为1.982,95％置信区间为1.029 ～2.837,P＜0.05;三酰甘油比值比为11.870,95％置信区间为2.819 ～14.822,P＜0.05;总胆固醇比值比为3.782,95％置信区间为1.673 ～ 8.993,P＜0.05.结论 ACS伴随有血清MMP-9含量的增加,并且与冠状动脉病变程度有一定的相关性,MMP-9、冠状动脉病变与脂质因子共同作用能促进ACS的发生.%Objective To investigate the correlation of matrix metalloproteinase-9 (MMP-9) expression and coronary artery disease in acute coronary syndrome(ACS) patients.Methods Totally 80 acute coronary syndrome patients from August 2011 to December 2013 in Hubei Medical College Affiliated Dongfeng Hospital were selected for the observation group (45 cases of unstable angina,35 cases of acute myocardial infarction),80 no-acute coronary syndrome patients were selected for the control group.The clinical data and biochemical data of two groups were analyzed retrospectively.Results The serum MMP-9 values in observation group was significantly higher than that in control group [(125 ± 14) μg/L vs (87 ± 14) μg/L,P ＜0.05],at the same time the serum MMP-9 values in patients with acute myocardial infarction were significantly higher than those in patients with stable angina [(146 ± 14) μg/L vs (111 ± 14) μg/L,P ＜0.05].Multivariate logistic regression analysis showed that the odds ratio of matrix MMP-9 was 1.982,the 95％ CI were 1.029-2.837,P ＜0.05 ; odds ratio of triglyceride was 11.870,95％ CI were 2.819-14.822,P ＜0.05 ; the odds ratio of total cholesterol was 3.782,the 95％ CI were 1.673-8.993,P ＜0.05.Conclusion ACS is associated with increased MMP-9 which is also correlated with severity of coronary artery disease.
