Objective To investigate and compare the curative efficacy of anti-rheumatic drugs combined with glucocorticoid or tumor necrosis factor antagonists in the treatment of refractory rheumatoid arthritis. Methods 90 patients with refractory rheumatoid arthritis were allocated to a control group and a treatment group, with 45 patients in each. The control group received the treatment of anti-rheumatic drugs combined with tumor necrosis factor antagonists; the treatment group received the treatment of anti-rheumatic drugs combined with glucocorticoid. Results The curative efficacy in the treatment group was significantly better than that in the control group;improvement of the ratio of Th17/regulatory T cells in the treatment group was significantly larger than that in the control group;symptoms alleviating time of arthritis and total drug treatment time were significantly shorter than those in the control group. Conclusion Anti-rheumatic drugs combined with tumor necrosis factor antagonists in the treatment of refractory rheumatoid arthritis has a significant effect.%目的：对应用抗风湿类药物与糖皮质激素或肿瘤坏死因子拮抗剂类药物联合对难治性类风湿性关节炎疾病实施治疗的效果进行对比研究。方法将90例难治性类风湿性关节炎患者分为对照组和治疗组，平均每组45例。采用抗风湿类药物与肿瘤坏死因子拮抗剂类药物联合对对照组患者实施治疗；采用抗风湿类药物与糖皮质激素类药物联合对治疗组患者实施治疗。结果治疗组患者难治性类风湿性关节炎疾病治疗效果明显优于对照组；Th17/调节性T细胞比值的改善幅度明显大于对照组；关节炎症状消失时间和用药治疗总时间明显短于对照组。结论应用抗风湿类药物与糖皮质激素剂类药物联合治疗难治性类风湿性关节炎效果明显。
