Objective To observe the effect of UTI combined with continuous hemofiltration on plasma inflammatory factors in patients with multiple organ dysfunction syndrome (MODS). Methods 46 patients with MODS were assigned to observation group (receiving UTI combined with continuous hemofiltration) and control group (receiving blood filtration only) according to order of admission. Changes of renal function, APACHEII scores, inflammatory cytokine interleukin-6 (IL-6), interleukin-8 (IL-8) and tumor necrosis factor α (TNF-α), length of hospital stay and continuous hemofiltration (CVVH) using time of the two groups were compared. Results Urea nitrogen (BUN) and creatinine (Scr) of both groups decreased after treatment and the decrease of the observation group were more significant (P < 0.05). Serum IL-6, IL-8 and TNF of both groups decreased after treatment and the decrease of the observation group were more significant (P < 0.05). And A-PACHEII score of both groups also decreased after treatment and the decrease of the observation group were more significant (P < 0.05). CVVH using and hospitalization time were significantly lower in the observation group than in the control group (P < 0.05). Conclusion CVVH combined with UTI can clear a variety of plasma inflammatory mediators in patients with MODS, and can also improve renal function and reduce hospital stay, thus it is worthy of clinical application.%目的 观察乌司他丁联合连续性血液滤过对多器官功能障碍综合征(MODS)患者血浆炎症因子的影响.方法 50例MODS患者依入院顺序分为观察组(乌司他丁联合连续性血液滤过)与对照组(血液滤过),比较两组的肾功能指标、炎症因子白介素-6(IL-6)、白介素-8(IL-8)、肿瘤坏死因子α(TNF-α)指标的变化及住院时间、连续性血液滤过(CVVH)的使用时间、APACHEⅡ评分变化情况.结果 两组患者的肾功能指标尿素氮(BUN)、肌酐(Scr)均较治疗前明显降低,且观察组较对照组降低更明显(P ＜ 0.05).两组患者血清中IL-6、IL-8、TNF-α治疗后均较治疗前明显降低,且观察组较对照组降低更明显(P ＜ 0.05).两组APACHEⅡ评分均较治疗前明显降低,且观察组较对照组降低更明显(P ＜ 0.05).观察组患者的CVVH使用时间、住院时间均明显低于对照组(P ＜ 0.05).结论 CVVH联合乌司他丁能清除MODS患者血浆多种炎症介质因子,并能改善患者的肾功能,缩短住院时间,值得推广和应用.
