Objective To investigate the therapeutic effect of three-dimensional conformal radiation therapy and intensity-modulated radiation therapy after the afterloading intracavitary radiotherapy for advanced cervical cancer.Methods 80 patients with advanced cervical cancer in Xingtai People's Hospital from April 2013 to April 2015 were selected.According to the different treatment methods,all the patients were divided into control group and observation group,with 40 cases in each group.The control group was treated by three-dimensional conformal radiotherapy + afterloading intracavitary radiotherapy,while the observation group was treated by intensity modulated radiation therapy (IMRT) + afterloading intracavitary radiotherapy.The short term effect,toxic and side effect and local recurrent rate,distant metastasis rate,overall survival after 2 years between two groups were compared.Results There was no statistically significant difference between the two groups in the short term effect (P ＞ 0.05).The blood elephant abnormal,digestive tract injury and urinary system iujury in the the observation group were lower than those in the control group,with highly statistically significant difference (P ＜ 0.01).There was no statistically significant difference between the two groups in the 2 years local recurrence,distant metastasis rate,survival rate of disease progression and overall survival rates (P ＞ 0.05).Conclusion There was no significant difference between the effect of radiotherapy and the long-term survival rate and the three dimensional conformal radiotherapy,but the incidence of toxicity was lower and the safety was higher.%目的 探究与分析三维适形放疗与调强放疗分别联合腔内后装放疗治疗中晚期宫颈癌的效果对比.方法 选取邢台市人民医院2013年4月～2015年4月收治的80例中晚期宫颈癌患者,根据不同治疗方法分为对照组与观察组,每组各40例.对照组给予三维适形放疗+腔内后装放疗,观察组给予调强放疗+腔内后装放疗,对比两组近期疗效、毒副作用及随访2年的局部复发率、远处转移率、无疾病进展生存率及总生存率.结果 两组近期总有效率比较,差异无统计学意义(P＞0.05).观察组血象异常、消化道损伤、泌尿系统损伤情况低于对照组,差异有高度统计学意义(P＜0.01).两组随访2年局部复发率、远处转移率、无疾病进展生存率及总生存率比较,差异无统计学意义(P＞0.05).结论 调强放疗治疗中晚期宫颈癌的效果及远期生存率与三维适形放疗无明显差异,但毒副作用发生率更低,安全性更高.
