DHS、PFNA、InterTan三种内固定术在老年股骨转子间骨折中的对比

             

摘要

目的:分析比较DHS、PFNA、InterTan三种内固定术在老年股骨转子间骨折中的应用效果。方法随机选取该院自2012年3月—2014年7月期间收治的90例老年股骨转子间骨折患者的临床资料,按照内固定方式的不同,将这90例患者分为3组,DHS组、PFNA组和InterTan组,3组各30例。分析比较3组患者的手术时间、术中出血量、下床活动时间、并发症情况等。结果 DHS组患者的手术时间(84.6±6.4)min以及术中出血量(371.4±6.8)mL均明显的高于PFNA组(61.9±5.7)min、(132.5±4.9) mL和InterTan组(65.3±7.1)min、(140.7±5.3)mL;PFNA组和InterTan组患者术后并发症的发生率(6.67%、10.00%)明显的低于DHS组患者术后并发症发生率(26.67%)差异有统计学意义;3组患者的下床活动时间差异无统计学意义。结论 PFNA、InterTan内固定术更加适用于病情不稳定的老年股骨转子间骨折的临床治疗PFNA、InterTan内固定术的抗旋转能力强以及手术时间短,并发症的发生几率小。%Objective To analysis and comparison the DHS and PFNA InterTan, three kinds of internal fixation in the ap-plication of the elderly femoral fractures between rotor effect. Methods Random selection rased on our hospital during March 2012 - July 2014 treated 90 cases of senile femoral fractures in patients with clinical data between rotor, according to the different methods of internal fixation, the 90 patients were divided into three groups, the DHS group and PFNA group and InterTan groups, three groups 30 cases each. Analysis and comparison of three groups of patients with operation time, intraoperative blood loss, ambulation time, complications, and so on. Results DHS group of patients with operation time (84.6±6.4) min and intraoperative blood loss (371.4±6.8) of ml were significantly higher than PFNA group (61.9±5.7) min and (132.5±4.9) mL and InterTan group(65.3±7.1) min and(140.7±5.3) mL; PFNA group and the incidence of postopera-tive complications in patients with the InterTan group (6.67%, 10.00%) obviously lower than that of DHS group of patients with postoperative complications (26.67%). Bed activity time of three groups of patients with no significant difference. Con-clusion PFNA, InterTan fixation is more applicable to the unstable condition of clinical treatment of senile femoral fractures between rotor-PFNA, InterTan fixation of rotation resistance is strong, shorter operation time and the risk of complications.
