针刺结合吞咽功能训练治疗脑卒中后（咽期）吞咽障碍临床疗效观察

摘要

目的：观察针刺结合吞咽功能训练对脑卒中后（咽期）吞咽困难的临床疗效。方法：选取脑卒中后吞咽困难的患者60例，按随机数字表法随机分为2组，每组30例。治疗组采用针刺疗法配合吞咽功能训练，针刺主穴取廉泉、金津、玉液、风池、合谷、太冲、通里，治疗时金津、玉液点刺放血，廉泉穴行强刺激后与患侧风池穴接通电针进行治疗，其余穴位采用平补平泻手法，每次留针30min，针刺治疗后予以吞咽功能训练，每周治疗5次，治疗6周共计30次；对照组仅采用吞咽功能训练，吞咽功能训练方法同治疗组，每周治疗5次，治疗6周共计30次。所有患者在治疗前后，均采用洼田氏饮水试验评价患者吞咽功能。结果：治疗组治愈8例，有效19例，无效3例，有效率为90％；对照组治愈2例，有效18例，无效10例，有效率为66．7％；治疗组与对照组相比较差异有显著性意义（ P＜0.05），且治疗组疗效优于对照组。结论：针刺结合吞咽功能训练是治疗脑卒中后（咽期）吞咽困难较为理想的方法，值得在临床推广应用。%Objective:To observe the curative effect of acupuncture therapy and swallowing functional rehabilitation to treat dysphagia after stroke .Methods:The patients who had acataposis because of acute cerebral accident were divided into two groups ( acupuncture group and control group ) .The patients of acupuncture group were given acupuncture and swallowing functional training of rehabilitation once a day and the patients in control group were given swallowing func -tional training only , then the curative effect had been observed after two courses of treatment .Results: Acupuncture group:There were 8 cases cured , 19 cases with approving results and 3 cases without any results .Control group:There were 2 cases cured , 18 cases with approving results and 10 cases without any results .Conclusion:Acupuncture therapy and swallowing functional training of rehabilitation have a reliable effect on dysphagia patients and it is a safe and effec -tive therapy without adverse reaction .
