Mechanisms of Pharmacists Interventions on Medication Management and Adherence in Patients with Type 2 Diabetes.

Optimal management of type 2 diabetes requires a collaborative, team-based approach; however, the role of pharmacists on these teams has not been well-defined. In the first study of this thesis, a systematic review examined components of pharmacist interventions to improve adherence to oral antidiabetic medications. The second study was a retrospective cohort analysis to determine how the pharmacist intervention in a randomized controlled trial achieved a significant improvement in blood pressure control.;Pharmacists can be effective additions to collaborative care teams by providing education-based interventions to improve adherence and helping to optimize medication regimens to achieve treatment targets in patients with type 2 diabetes.;The systematic review identified that pharmacist interventions to improve adherence include educational strategies combined with behavioural, affective, or provider-targeted strategies. In the cohort study, the observed improvement in blood pressure was likely due to pharmacist interventions to optimize antihypertensive medication management rather than improve medication adherence.
