A case study in project management: Redesign of a family health center to meet patient centered medical home.

Primary care offices are expected to provide patient centered comprehensive health care to maintain solvency in today's competitive market. The primary care practices that meet the National Committee for Quality Assurance (NCQA) recognition standards as Patient Centered Medical Home (PCMH) will provide quality health care to their patients while reaping the financial benefits of payment and performance initiatives sponsored by private and public payers. PCMH transformation benefits focus on improving patient experience, reducing clinical burnout, reducing hospitalization rates, reducing emergency room visits, and providing a higher quality of health care at lower cost. The goal of this applied research project is to redesign the North Country Family Health Center's practice model to meet PCMH based on the 2011 standards as recognized through NCQA. The three to nine month re-design process will focus on following the nine steps to PCMH recognition and the four phases of project management: Planning, Build Up, Implementation, and Close out. The PCMH team will evaluate patient satisfaction and quality indicators based on the quality improvement plan, with a goal to improve patient outcomes by increasing productivity and collaboration within its primary care practice, all while reducing health care costs.
