Implementation of a discharge protocol for adult psychiatric inpatients.

Psychiatric treatment has changed drastically, as deinstitutionalization has led to an increased number of individuals being discharged into the community. Additionally, psychiatric hospitals are experiencing a revolving door phenomenon, where individuals are readmitted to the hospital within 30 days. Individuals discharged from psychiatric facilities are ill-prepared for discharge and mental illness limits individuals' ability to be self-sufficient, increasing their need for structure, assistance, and community support/resources. Nurses can utilize high-quality education materials to combat this phenomenon. Therefore, the purpose of this clinical project was to improve psychiatric inpatient discharge readiness and prevent psychiatric readmission. The Chronic Care Model was the theoretical framework utilized, which focused on improving outcomes for patients living with a chronic disease. There were two overall goals, develop and implement a discharge protocol and utilize the teach-back method during the discharge process. These goals were both successfully met by the end of the four-month implementation timeframe.
