Patterns of early childhood caries (ECC) have been described utilizing a priori pattern definitions. The purpose of this project was to answer the question "are there distinct patterns of dental caries in early childhood?" utilizing analyses conducted without a priori pattern delineation. The importance of this project is that caries patterns (if real) should (1) help reduce nondifferential disease misclassification and thereby (2) enhance ability of an analysis to identify meaningful associations between suspected risk factors and ECC.; Clinical and demographic data on 5,169 Arizona children ages 5--59-months were analyzed for this project. Median primary tooth eruption ages for Hispanic and White (non-Hispanic) children were produced from logistic regression parameter estimates. Caries patterns were classified by modeling tooth surface level caries data utilizing multidimensional, scaling and cluster analyses. Each of the identified caries patterns was analyzed for associations in regards to both ethnicity and social-economic status (SES). These patterns were applied in assessing the validity and utility of a recently proposed (NIDCR) ECC case definition using classification and regression tree, as well as sensitivity and specificity analyses.; Results for contemporary median eruption ages of the primary teeth were produced. Classification analyses supported the existence of the following ECC caries patterns: (1) smooth surfaces, (2) maxillary incisor surfaces, (3) first molar occlusal surfaces, and (4) second molar pit and fissure surfaces. The individual patterns demonstrated differential associations with ethnicity and SES levels. The smooth and maxillary incisor surface patterns were both consistent with the NIDCR workshop's severe-ECC caries level (dmfs) criteria; however, these two patterns, while demonstrating good sensitivity for the dmfs criteria, have poor specificity.; We conclude that patterns of dental caries exist in the primary dentition of pre-school children and are differentially associated with SES and ethnicity. These patterns may, result, in part, from different tooth eruption ages. We also conclude that the NIDCR workshop's definition of ECC should be modified such that either the patterns of smooth or maxillary incisor surfaces define severe-ECC at any age. This new criterion has greater utility in population studies than at the individual level.
