In this prospective study of 34 patients awaiting Roux-en-Y gastric bypass surgery the best predictors of measured resting energy expenditure (MREE) and the precision of predictive equations were examined. Height, weight, age, gender, race, and 24-hour recall were collected. REE was measured and calculated using Harris-Benedict (HB), Bernstein, and World Health Organization equations. A significant (p = 0.04) three-way interaction between BMI, age, and gender was observed for MREE. MREE was lower for BMI 50 (2095 +/- 379 kcal/day) than BMI ≥ 50 (2505 +/- 405 kcal/day; p = 0.005). The HB equation using actual weight most strongly correlated (r = 0.74; p 0.0001) with MREE, the difference was 86 kcal. Caucasians had higher MREE (2345 +/- 548 kcal/day; p = 0.05) than African Americans/Hispanics (2243 +/- 337 kcal/day). The HB equation may be used to estimate REE; BMI, gender and age should be considered in this estimate.
