Background: Conflicting results of available animal studies preclude making a definitive statement on the role of bone marrow penetration in guided bone regeneration (GBR) procedures. The purpose of this study was to clinically evaluate and compare dimensional changes in ridge width and height after alveolar ridge augmentation with and without bone marrow penetration.;Methods: 21 patients with horizontal ridge deficiency were randomly assigned into one of two groups. GBR procedures were performed with bone marrow penetration (test, n=11) or without (control, n=10). Surgical measurements included ridge width (at 3mm, 6mm, and 9mm below the bone crest) and ridge height using custom-made stent during GBR surgery and 4-6 months post-surgically.;Results: 19 patients (19 sites) completed the study. With GBR procedures, the alveolar ridge width increased significantly compared to baseline at all levels with or without bone marrow penetration (p<0.05). The median horizontal ridge width increase at 3, 6, 9 mm level respectively were 2.00 mm, 1.50 mm and 1.00 mm in the test group versus 3.00 mm, 2.50 mm and 0.63 mm in the control group, while the vertical ridge height increase was 0.50 mm in the test group versus 0.75 mm in the control group. No statistically significant difference between the two treatment modalities was found for either ridge width or ridge height change (p>0.05).;Conclusion: Both treatments produced significant clinical improvements in alveolar ridge width within the groups. Intergroup comparison revealed no statistically significant difference between the two treatment modalities in either ridge width or ridge height change.
