End-stage renal disease patients and dialysis: Can consistent transportation influence quality of life and treatment compliance? A grant writing project.

The purpose of this project was to write a grant to fund a specialized transportation program for elderly people living with end-stage renal disease (ESRD) in Ventura County in collaboration with Senior Concerns. Senior Concerns is a private, nonprofit organization that assists frail and special-need seniors, caregivers, and families by providing quality programs, appropriate resources, and educational outreach. The program will provide a means of transportation for dialysis recipients 65 years and older who have compromised transportation availability or no transportation available to them at all.;Based on the findings from the literature review, dialysis treatments become a lifeline for ESRD patients; therefore, reliable transportation is necessary to ensure that patients have access to dialysis centers. After an extensive review of funding sources, the funding source selected was the California Wellness Community which supports healthy aging. The goals of the program include increased treatment compliance and improved quality of life.
