This study showed that a mass spectrometry based proteomic analysis could be a useful tool to get information of pancreatic cyst fluid. Our novel approach including retention of in-soluble pellet and high-pH fractionation using in-house StageTip made it possible to discover 5,002 proteins in very small amount of pancreatic cyst fluid. Despite the clinical sample deviations, we could found hundreds of low abundant proteins as well as tumor marker proteins. When viewed as a reproducible results, our proteomic technic seems appropriate to the clinical sample analysis. In addition, this results can further utilized in large scale study that aimed to find biomarkers distinguishing IPMN from Mucinous Cystic Neoplasm (MCN), Pseudocyst (PC) or etc. Moreover, a total of 590 mutation sites were identified in this study, of which 19 variances were occurred in 9 pancreatic tumor marker proteins. Global mutation profiling using mass spectrometry like this work still needs somewhat manual inspection due to false positives. Provided that more convenient and automated software are developed, proteomics technic can be a complementary method in proteogenomic research for mutation discovery.
