A Potential Failure Modes Assessment (PFMA) identified 63 independent potential failure modes (PFMs) at both the Auxiliary (Aux) and Main Dams and appurtenant structures. Twenty two of these PFMs were judged to be both credible and highlighted for detailed risk evaluation.Four of the most significant failure modes have been described in this paper. Two are associated with seepage and piping in the Auxiliary Dam foundation including a location adjacent to the Borel Canal Conduit, and two are associated with the response of the Auxiliary Dam to seismic induced ground motions and fault rupture. Hydrologic overtopping was also identified as a significant failure mode; however, it was not included in this paper.
