Radiation Exposure to Patients and Physicians duringCT Fluoroscopy-guided Interventional Procedures

Purpose: We assessed the radiation dose to the patient and the physician during fluoroscopy-guided radiology procedureusing x-ray computed tomography.Methods: 1) Peak skin doses were assessed for 18 patients at two institutes. (1) Marking for video-assisted thoracoscopicsurgery (n=10), (2) needle biopsy of lung masses (n=5), (3) biopsy of neoplasm in the pelvis and mesentery (n=3). Weestimated the peak entrance surface doses (ESDs) of the patients using thermoluminescent dosimeters (TLD). 2) Doses tophysicians were assessed for 17 procedures at the same time.3). The effective dose was estimated using the record of the X rayCT fluoroscopy. The relation between the dose to the patient's skin and around the patient was calculated.Results: Maximum skin dose of patient was ranged 210 - 1 mGy (median, 37mGy). Maximum effective dose to physicianwas 0.08mSv(median, 0.03mSv), and maximum dose was 1.2mSv(median, 0.01mSv) at the finger. The dose outside theprimary beam in 5cm distance from the patient surface was 14±1% compared with the skin dose of patient. Ring dosimetersmight possibly underestimate doses because of the geometric condition and attenuation in the finger.Conclusion: The doses to patients in these procedures were acceptable. The doses to hands of physicians were substantiallyless than dose limits.
