This paper will discuss the role of human error in theincident causation process and indicate what the mosteffective means to identify and manage human error.Human is an important contributing cause in up to atleast 90% of all incidents. Consequently the eliminationof human error should be a most promising target forincident prevention. Human error preventionconcentrates on systemic factors and the way in whichmanagement decisions can propagate into substandardconditions and actions at the work place. It attempts tohelp the organization to understand and control the incident causation process and not to focus mainly on theindividual worker or the negative outcomes of incidents.Optimal control of the controllable workplace conditionsthat cause human error makes an organization maximallyintrinsically safe. Managing these conditions is the nextstep in getting as close to zero incidents as economicallyand logistically feasible.
