METHOD FOR SURGICAL TREATMENT OF GIANT PENETRATING ULCERS OF POSTERIOR DUODENAL WALL COMPLICATED WITH PROFUSE HEMORRHAGE AT THE BACKGROUND OF HEMORRHAGIC SHOCK II-III DEGREE

FIELD: medicine, surgical gastroenterology.;SUBSTANCE: the complications could be induced by profuse hemorrhage at the background of hemorrhagic shock or other critical life-threatening states, also, caused by accompanied pathologies. One should suture a bleeding vessel with a Z-shaped hemostatic suture. With the help of 3-5 layers of hemostatic sponge, its size exceeding the diameter of ulcerous defect by 5-8 mm, up to the level of submucous layer it is necessary to fulfill tamponade of the ulcerous defect. The edges of the ulcer should be coincided for the distance being about 12-15 mm, not less due to applying uninterrupted suture with nonabsorbable ligature upon atraumatic needle. The suture should be applied beyond submucous layer of ulcerous walls at pace width being about 5-7 mm. The innovation enables to create conditions for final surgical hemostasis, efficient reparative process, ulcerous self-healing without subsequent operations.;EFFECT: higher efficiency of therapy.;1 dwg, 1 ex
