The authors have consistently striven to explore the interest and participation of all health care entities, including Medco, in the area of appropriate medication therapy, medication errors, and disease management. We also have made every attempt to maintain accuracy in our study and reported metrics. That being said, we realize that we are a part of a rapidly changing industry that continues to upgrade and provide better services to patients worldwide. The study was conducted in the 2003-2005 time frame and the comments were based on personal interviews with personnel directly or indirectly involved with medication error and disease management programs.
