A previously healthy 51-year-old man suddenly fell to the ground while singing karaoke. On arrival to the emergency department, he had a Glasgow Coma Scale score of 4, with symmetric and reactive pupils, a blood pressure of 149/104 mm Hg, pulse of 105 beats/min, respiratory rate of 26 breaths/min, and SpO_2 of 91% on room air. He received prompt intubation, and a head computed tomography (CT) scan demonstrated diffuse subarachnoid hemorrhage (Figure 1). His respiratory status then further deteriorated. Pink frothy sputum was suctioned through the endotracheal tube, SaO_2 was 84% (100% FiO_2), and a chest radiograph was obtained (Figure 2).
