Acetic acid iontophoresis for recalcitrant scarring in post-operative

It is with great interest that we read the article by Agnes Dardas et al, "Acetic acid iontophoresis for recalcitrant scarring in postoperative hand patients" published in Journal of Hand Therapy} We commend the authors on the time and effort undertaken to publish this article which suggested that acetic acid iontophoresis (AAI) is an effective treatment modality significantly improved the functionality of hand in post-operative patients. In this study, 23 digits in 17 patients having undergone an open trigger finger release and followed up exclusively at the outside hand therapy facility. All 23 fingers were treated with standard of care (SOC) occupational therapy (OT) management, and the 10 fingers that did not respond well to SOC were given additional treatment with AAI. The result showed that the SOC + AAI treatment allowed patients whose total active range of motion (TAM) improvement prematurely stopped with only SOC to reach a functional recovery similar to the patients who responded very well to SOC. The authors postulated that the great increase in TAM for patients after AAI may be due to the remodeling of the collagen fibers in scars. However, we still have some supplements and questions on the viewpoint.
