A 44-year-old woman was admitted to the hospital in a severely impaired mental state characterized by stupor. The patient had complained of suffering from headaches and vomiting for 2 days. We found no evidence of immune deficiency, including infection with the human immunodeficiency virus, malignancy, or drug-related reactions. No history of other medical illness was reported.Diffusion-weighted images obtained upon admission showed multiple high-intensity lesions with reduced apparent diffusion coefficients (Fig. 1A) involving the cortical and subcortical regions (Fig. IB). These manifested as ring-enhancing cystic lesions without surrounding oedema on the gadolinium-enhanced T2-weighted MRI sequence and the gadolinium-enhanced T1-weighted MRI (Fig. 1C and D).
