These events reveal that facility criticality safety staff must pay close attention to all procedures at their facilities in order to detect when fissile operations are being performed. In the case of the fuel rods moved outside of criticality alarm coverage, facility criticality safety staff were aware of the operation but were unaware of the method used to move the rods.
