A vivid example of this is described in a casualty information report issued last year by Norwegian classification society DNV. The circumstances of the particular incident were such that DNV felt that potentially they could arise on almost any vessel at any time and that the wider maritime world would benefit from learning from them. On the ship in question the engine room bilges became flooded because of a leaking seawater pipe, but the situation was made worse because proper actions were not taken when the alarm sounded. As a result, the water had already reached the engine room floor plates by the time the leakage was discovered.
