微小RNA-17在幽门螺杆菌相关胃癌组织中的表达变化及意义

         

摘要

Objective To observe the expression change of microRNA-17 in the helicobacter pylori (Hp) -associated gastric cancer tissues and its clinical significance.MethodsSeventy-five ceases of gastric cancer tissue specimens and their matched adjacent tissue specimens from patients with gastric cancer were collected, including 48 patients with hp-positive gastric cancer and 27 patients with hp-negative gastric cancer.Real-time quantitative PCR was used to determine the expression levels of cytotoxin-related gene A (Cag A) mRNA and miR-17.Results The expression levels of Cag A mRNA in Hp-positive gastric cancer tissues (6.061±0.712) was significantly higher than that in the Hp-negative gastric cancer patients (1.024±0.235) (t=35.593, P<0.05).The miR-17 expression levels of gastric cancer tissues and adjacent tissues in patients with Hp-positive gastric cancer were significantly higher than those in patients with Hp-negative gastric cancer (12.785±1.324 vs 5.654±0.745, 8.124±1.105 vs 3.124±0.564) (t=23.739, 21.911, P<0.01).The miR-17 expression level of gastric cancer tissues in the Hp-positive and Hp-negative patients was significantly higher than that in the adjacent tissues (12.785±1.324 vs 8.124±1.105, 5.654±0.745 vs 3.124±0.564) (t=18.725, 14.069, P<0.01).In the gastric cancer tissues, miR-17 expression was positively correlated with Cag A expression (r=0.564, P<0.01).The level of miR-17 expression of gastric cancer in patients with tumor diameter>5cm, TNM stagingⅢ-Ⅳ, Borrmann typeⅢ-Ⅳand lymph node metastasis Hp infection was significantly higher than that of patients with tumor diameter less≤5 cm, TNM stageⅠ-Ⅱ, Borrmann typing-Ⅱ, non-lymph node metastasis (15.226±1.325 vs 11.041±1.214, 14.485±1.412 vs 11.571±1.1.16, 14.341±1.325 vs 11.468±1.241, 15.124±1.332 vs 9.778±1.123), with statistically significant difference (t=11.336, 7.983, 7.748, and 7.748, all P<0.05).Conclusion The abnormal expression of miR-17 in gastric cancer tissues with Hp infection is significantly related to tumor diameter, TNM staging, Borrmann typing, lymph node metastasis, and prognosis, which can be used as a candidate marker for early diagnosis and prognosis of gastric cancer.%目的 观察微小RNA-17(miR-17)在幽门螺杆菌(Hp)相关胃癌组织中的表达变化,并探讨其临床意义.方法 收集75例份胃癌组织及其配对的癌旁组织标本,其中Hp阳性48例份、Hp阴性27例份.采用实时定量PCR法检测细胞毒素相关基因A(Cag A)mRNA、miR-17表达.结果 Hp阳性胃癌组织Cag A mRNA表达为6.061±0.712,Hp阴性胃癌组织为1.024±0.235,二者比较差异有统计学意义(t=35.593,P<0.05);Hp阳性胃癌组织及其癌旁组织miR-17表达均明显高于Hp阴性胃癌组织及其癌旁组织(12.785±1.324 vs 5.654±0.745,8.124±1.105 vs 3.124±0.564),二者比较差异均有统计学意义(t分别为23.739、21.911,P均<0.01);Hp阳性、阴性胃癌组织miR-17表达均高于癌旁组织(12.785±1.324 vs 8.124±1.105,5.654±0.745 vs 3.124±0.564),二者比较差异均有统计学意义(t分别为18.725、14.069,P均<0.01);胃癌组织miR-17表达与Cag A表达呈正相关关系(r=0.564,P<0.01);肿瘤直径>5 cm,临床分期Ⅲ、Ⅳ期,Borrmann分型Ⅲ、Ⅳ型以及有淋巴结转移的Hp阳性胃癌组织miR-17表达高于肿瘤直径≤5 cm,临床分期Ⅰ、Ⅱ期,Borrmann分型Ⅰ、Ⅱ型以及无淋巴结转移的Hp阳性胃癌组织(15.226±1.325 vs 11.041±1.214,14.485±1.412 vs 11.571±1.116,14.341±1.325 vs 11.468±1.241,15.124±1.332 vs 9.778±1.123),二者比较差异均有统计学意义(t分别为11.336、7.983、7.748、14.753,P均<0.05).miR-17高表达Hp阳性胃癌患者平均生存时间明显短于miR-17低表达Hp阳性胃癌患者(t=5.170,P<0.05).结论 miR-17在Hp相关胃癌组织中的表达异常升高,且与肿瘤直径、临床分期、Borrmann分型、淋巴结转移及患者预后有关,可作为胃癌早期诊断及预后判断的候选标志物.
