(足母)外翻足第1跖序列的X线影像学观察

         

摘要

Objective To observe the pathological structure characterize of the hallux valgus foot, and study the pos sible etiological mechanism. Methods 211 feet of 121 patients in the hallux valgus group, 35 feet of 29 healthy volunteers in the control group. Aquired the hallux valgus angle(HAV) , inter-metatarsal angle (IMA) from these radiographes. The two-dimentional coordemite mapping system was used for marking the anatomical points on the weight-bearing doreoplantar radiograph. Results Compare with the feet in control group, hallux and second toe were obviously lateral deviation in hal lux valgus group. The midpoint of the base of first proximal phlangeal was lateral translation while the HAV > 50° , no simi lar changes were found while the HAV < 50°. Medial translation of the first metatarsal head were found in in hallux valgus group. The medial point of proximal base of first metatarsal, and the distal articulate surface of internal cuneiform are loca ted in more distally positions from proximal second metatarsal base in foot sagittal axial. Conclusion Metatarsal primus varus is a major factor in the development of hallux valgus, which is a particular pathological structure character of hallux valgus foot.%目的 观察母外翻足的病理形态,探讨其可能的发病机制.方法 (足母)外翻畸形患者((足母)外翻组)121例(211足),足部外观正常、无任何不适的健康志愿者(对照组)29例(35足).两组均摄负重位足部正位X线片,测量母外翻角(HAV)及第1、2跖骨间角(IMA),用二维坐标系对各解剖标记点进行定位.结果 与对照组相比,(足母)外翻组的(足母)趾向外侧发生明显偏斜,近节趾骨基底中点在HAV<50°时无变化、HAV> 50.时则发生外移,(足母)外翻组的第1跖骨头内移.(足母)外翻组的第1跖骨基底中点及内侧楔骨远端关节面的内外侧标记点较对照组的相应点在足纵轴方向远离第2跖骨基底.结论 (足母)外翻足有其特殊的病理形态,其发病机制为第1跖骨内翻.
