电刺激小脑顶核及奥拉西坦治疗非痴呆性血管性认知障碍的疗效比较

摘要

目的 观察电刺激小脑顶核(FNS)及奥拉西坦治疗非痴呆性血管性认知障碍(VCIND)患者的临床疗效.方法 选择60例VCIND患者,随机分为FNS组和奥拉西坦组各30例.在常规药物治疗的基础上,FNS组给予FNS治疗,奥拉西坦组给予口服奥拉西坦治疗.分别观察两组治疗前后事件相关电位-P300 (ERP-P300)及蒙特利尔认知评估量表(MoCA)评分的变化.结果 两组治疗后MoCA评分均较治疗前明显升高(P均＜0.01),且FNS组升高更显著(P＜0.01)；两组治疗后均有ERP-P300潜伏期缩短、波幅增高(P均＜0.01),FNS组治疗后潜伏期缩短较奥拉西坦组更明显(P＜0.01),而奥拉西坦组治疗后波幅增高较FNS组更显著(P＜0.01).结论 FNS、奥拉西坦均可改善VCIND患者的认知功能,但FNS对提高患者的执行能力更有效.%Objective To investigate the clinical effects of the treatment of the cerebellar fastigial nucleus electrical stimulation (FNS) and oxiracetam for the treatment of vascular cognitive impairment not dementia (VCIND). Methods A total of 60 patients with VCIND were randomly divided into FNS group and oxiracetam group, each 30 cases. On the base of receiving conventional treatment in all groups, FNS was applied in the FNS group and the treatment of oxiracetam in the oxiracetam group. The event-related potentials P300 ( ERP-P300) and montreal cognitive assessment ( MoCA) of all pa-tients were observed and compared before and after the treatment. Results After the treatment, the scores of MoCA of the FNS group and the oxiracetam group had been improved (P< 0.01). The improvement of the scores of the FNS group was more obvious than that in the the oxiracetam group (P < 0.01). After the treatment, the P300 of the FNS group and the Oxiracetam group had been improved, the latency period of P300 was significantly shorten and the amplitude were signifi-cantly increased (P <0.01). The improvement of the latency of P300 in the FNS group was more obvious than that in the Oxiracetam group ( P < 0.01) , but the improvement of the amplitude inthe oxiracetam group was more obvious than that in the FNS group (P <0. 01). Conclusions The FNS, oxiracetam can both improve the cognitive function of the patients with VCIND. The FNS can effectively improve executive function in patients with VCIND.
