IVF-ET不同妊娠结局患者子宫内膜蠕动方式及血流分型分析

         

摘要

Objective To analyze in vitro fertilization-embiyo transfer (IVF-ET) different pregnancy outcome of patients with endometrial movement method and blood flow typing. Methods To select 92 cases implementing IVF-ET infertility patients as the research object, according to its pregnancy outcome they were divided into biochemical pregnant group and the non-pregnant group. The endometrial movement method and the inner flow distribution were observed respectively on the day of HCG and on the day of ET in transvaginal color doppler sonography. Results On the HCG day, the biochemical pregnant group of endometrial movement method had the positive movement 18 cases, the negative movement 15 cases, the counter movement 12 cases, the irregular movement 4 cases, the no movement 1 case. The movement frequency for the dynamic were 23 cases, 27 cases for the small moving. The non-pregnant group respectively were 8, 16, 11, 6, 1, 18 and 24 cases, P > 0.05. The biochemical pregnant group of endometrial blood blow typing respectively were in type A 0 case, type B 25 cases, type C 25 cases, the non-pregnant group respectively were 12, 24 and 6 cases, P<0.05. On the ET day, the biochemical pregnant group of endometrial peristalsis way had the positive movement 18 cases, the negative movement 5 cases, the counter movement 4 cases, the irregular movement 4 cases, the no movement 19 cases, and the movement frequency for large dynamic was 0 case, the dynamic 8 cases, the small moving 42 cases, the non-pregnant group respectively were 5, 16, 11,6,4,1, 30, 11 cases, P < 0.05. The biochemical pregnant group of endometrial blood blow typing respectively were in type A 3 cases, type B 23 cases, type C 24 cases,the non-pregnant group respectively were 13, 20 and 9 cases, P <0.05. Conclusions The endometrial movement on the day of HCG has no influence on pregnant outcome, while the endometrial movement method on the day of ET is closely related to pregnant outcome. If it is held that there is positive movement or relative rest movement on the day of ET in slight movement form, the endometrial receptivity is better which is apt to nidation. The endometrial blood flow typing is significant in evaluation of endometrial receptivity to certain extent. Type B and type C are dominant in pregnant group, while type A and type B are dominant in non-pregnant group.%目的 分析体外受精—胚胎移植(IVF-ET)不同妊娠结局患者的子宫内膜蠕动方式及血流分型.方法 选取92例实施IVF-ET的不孕患者作为研究对象,根据其妊娠结局分为生化妊娠组与非妊娠组;分别于注射人绒毛膜促性腺激素(HCG)日和胚胎移植(ET)日,用经阴道彩色多普勒超声诊断仪观察患者子宫内膜蠕动方式及内膜下血流分布.结果 HCG日生化妊娠组子宫内膜蠕动方式为正向运动18例、负向运动15例、相向运动12例、不规则运动4例、无运动1例,蠕动频率为中动23例、小动27例;非妊娠组分别为8、16、11、6、1及18、24例.两组比较,P均>0.05.生化妊娠组子宫内膜血流分型A型0例、B型25例、C型25例,非妊娠组分别为12、24、6例;两组比较,P均<0.05.ET日生化妊娠组子宫内膜蠕动方式为正向运动18例、负向运动5例、相向运动4例、不规则运动4例、无运动19例,蠕动频率为大动0例、中动8例、小动42例;非妊娠组分别为5、16、11、6、4、1、30、11例.两组比较,P均<0.05.生化妊娠组子宫内膜血流分型A型3例、B型23例、C型24例,非妊娠组分别为13、20、9例;两组比较,P均<0.05.结论 HCG日内膜蠕动方式对妊娠结局影响不大,而ET日内膜蠕动方式与妊娠结局密切相关;ET日正向运动或相对静止型运动且运动为小动者,子宫内膜容受性较好,易于孕卵着床;内膜血流分型对子宫内膜容受性的评价有一定意义,妊娠者以B、C型为主,非妊娠者以A、B型为主.
