单侧星状神经节切除对家犬房颤诱发、心房电生理活动的影响

         

摘要

目的:观察单侧星状神经节( SG)切除对家犬房颤诱发、心房电生理活动的影响。方法将16只家犬随机分为对照组(4只)、RSG组(6只)、LSG组(6只)。对照组给予单纯快速心房起搏,不刺激SG;RSG组给予单纯快速心房起搏,刺激右侧SG;LSG组给予单纯快速心房起搏,刺激左侧SG;6 h后LSG组切除左侧SG,RSG组切除右侧SG。应用心电生理仪测算刺激SG前、后及切除SG后右心房( RA)、左心房( LA)、左上肺静脉( LSPV)、左下肺静脉( LIPV)部位的房颤诱发率、房颤持续时间,使用心脏电生理刺激仪测定刺激SG前、刺激后2、4、6 h及切除SG后RA、LA、LSPV、LIPV部位的有效不应期(ERP)和不应期离散度(dERP)。结果 RSG组刺激SG前后及切除SG后RA部位的房颤诱发率分别为25.00%、73.30%、31.30%,两两比较,P均<0.05。 LSG组SG刺激前LA、LSPV、LIPV部位的房颤诱发率分别为27.10%、33.30%、18.80%,刺激后分别为63.00%、70.80%、47.90%,SG切除后分别为35.40%、39.60%、25.00%,刺激SG前后及切除SG后LA、LSPV、LIPV的房颤诱发率比较,P均<0.05。 RSG组刺激SG前后及切除SG后RA房颤持续时间分别为(20.64±1.76)、(76.47±2.23)、(25.12±4.67)s,两两比较,P均<0.05。 LSG组SG刺激前LA、LSPV、LIPV房颤持续时间分别为(23.75±1.88)、(20.80±3.60)、(25.31±1.52) s,刺激后分别为(92.44±1.91)、(81.72±3.03)、(66.39±4.76)s,切除后分别为(30.47±5.25)、(38.32±4.12)、(33.45±3.11)s,刺激SG前后及切除SG后LA、LSPV、LIPV的房颤持续时间比较,P均<0.05。 RSG、LSG组SG刺激2、4、6 h时RA部位的ERP均显著短于基础值,dERP显著高于基础值(P<0.05),而SG切除后ERP显著高于、dERP显著低于刺激时(P<0.05)。结论单侧SG切除能明显降低家犬房颤的诱发率,并且能逆转SG刺激后的心房电生理活动变化。%Objective To investigate the effect of unilateral stellate ganglion(SG) excision on atrial fibrillation(AF) in-ducibility and atrial electrophysiological properties of mongrel dogs.Methods Sixteen mongrel dogs were randomly divived into 3 groups:control group(n=4), RSG group(n=6), LSG group(n=6).Control group underwent simple rapid atrial pacing, without stimulating the SG.RSG group was given simple rapid atrial pacing, without stimulating the right SG.LSG group under-went simple rapid atrial pacing, without stimulating the left SG.After 6 h, left side SG was removed in LSG group, and in RSG group, the right SG was removed.ECG physiological instrument was used to measure the rate of AF inducibility, AF duration of RA, LA, LSPV, LIPV respectively before and after stimulating the SG and after removing the SG.Using ECG physiological in-strument to measure the ERP and dERP of RA, LA, LSPV, LIPV before stimulating the SG, 2, 4, 6 h after stimulating and re-moving the SG.Results In RSG group, before and after stimulating SG and after removing SG, the induction rate of AF of RA was respectively 25.00%, 73.30%, 31.30%, compared with each other , all P<0.05.In LSG group, before stimulating the SG, the induction rate of AF of LA, LSPV, LIPV was respectively 27.10%, 33.30%, 18.80%, after stimulating, the rate was 63.00%, 70.80%, 47.90%, respectively, after removing the SG, the rate was 35.40%, 39.60%, 25.00%, respectively.Com-pared with the induction rate of AF before and after stimulating the SG and after removing the SG in LA, LSPV, LIPV, P<0.05.In RSG group, the AF duration of RA before and after stimulating the SG and removing the SG was respectively (20.64 ± 1.76), (76.47 ±2.23), (25.12 ±4.67)s, compared with each other, all P<0.05.In LSG group, the AF duration of LA, LSPV, LIPV before stimulating the SG was respectively (23.75 ±1.88), (20.80 ±3.60), (25.31 ±1.52)s, after stimulating the SG, the duration was (92.44 ±1.91), (81.72 ±3.03), (66.39 ±4.76)s, respectively.After removing the SG, the duration was respectively (30.47 ±5.25), (38.32 ±4.12), (33.45 ±3.11)s.Compared with the AF duration before and after stimulating the SG and after removing the SG in LA, LSPV, LIPV, all P<0.05.In RSG and LSG group, 2, 4, 6 h after stimulating the SG, the ERP of RA was significantly shorter than those on baseline, dERP was significantly longer than that on baseline(P<0.05), but after removing the SG, ERP was significantly longer, dERP was significantly shorter than that when stimulating(P<0.05). Conclusion Removing unilateral stellate ganglion can reduce the AF induction of mongrel dogs significantly, and can reverse e-lectrical activity changes of atrium after stimulating the SG.
