目的：观察不同剂量培哚普利对老年原发性高血压患者血压及肾功能的影响。方法选择老年原发性高血压患者254例，随机分为培哚普利低剂量组、培哚普利高剂量组各127例，分别给予培哚普利4、8 mg、1次/d，连续治疗6个月。观察两组治疗前后血压、肾功能及24 h尿微量白蛋白变化。结果培哚普利低剂量组降压总有效率为95．27％、培哚普利高剂量组为97．64％，两组比较P＞0．05。治疗6个月后，两组血压、血肌酐、尿素氮、24 h尿微量白蛋白较治疗前均有不同程度改善（P＜0．05或＜0．01），且培哚普利高剂量组较培哚普利低剂量组改善更明显（P均＜0．05）。结论培哚普利对老年原发性高血压患者降压效果确切，并能改善肾功能，高剂量（8 mg）效果更好。%Objective To investigate the effects of different doses of perindopril on blood pressure and renal function in patients with primary hypertension .Methods Totally 254 elderly patients with primary hypertension were randomly di-vided into two groups:the low-dose perindopril group and high-dose perindopril group .The patients in the low-dose group were treated with perindopril 4 mg, once a day; and patients in the high-dose group were treated with perindopril 8 mg, once a day.The treatment time of the two groups lasted for 6 months.Blood pressure, renal function and 24 h urinary albu-min quantification were observed in the two groups before and after treatment .Results The total efficiency of antihyperten-sive effect in the low-dose group was 95.27%, and 97.64% in the high-dose group (P>0.05).After treatment of 6 months, the serum creatinine, blood urea nitrogen and 24 h urine albumin decreased significantly after treatment of perindo-pril (all P<0.05);and the changes were more significant in the high-dose group than that of the low-dose group (all P<0.05).Conclusions Perindopril has exact antihypertensive effect in patients with primary hypertension , and it can im-prove renal function.Meanwhile, the antihypertensive effect of the high-dose group (8 mg) is better.
