噻托溴铵治疗稳定期中重度慢性阻塞性肺疾病的效果观察

摘要

目的：评价噻托溴铵治疗稳定期中重度慢性阻塞性肺疾病（ COPD ）患者的临床效果。方法选择稳定期中重度COPD患者56例，随机分为观察组、对照组各28例。对照组给予福莫特罗/布地奈德吸入治疗，观察组给予噻托溴铵联合布地奈德吸入治疗，疗程均为48周。比较两组治疗前后肺功能变化、急性加重次数、间隔时间、呼吸困难评分、生活质量评分。结果观察组治疗48周后，第1秒用力呼气量（ FEV1）、用力肺活量（ FVC）、FEV1/FVC及深吸气量较治疗前明显改善（P均＜0．05），且各指标均优于对照组治疗后（P均＜0．05）；两组治疗后急性加重次数、间隔时间、生活质量评分、呼吸困难评分较治疗前明显改善（P均＜0．05），且观察组改善更明显（P均＜0．05）。结论噻托溴铵治疗中重度稳定期COPD患者临床效果较好，值得在临床上推广应用。%Objective To evaluate the clinical efficacy of inhaled tiotropium in treatment of patients with moderate to severe stable chronic obstructive pulmonary disease (COPD).Methods Fifty-six patients with moderate to severe COPD in stable phase were randomly divided into the observation group and control group , 28 in each group.Patients in the con-trol group were treated with symbicort turbuhaler , and patients in the observation group received tiotropium combined budesonide therapy.Both the courses of treatment were 48 weeks.The pulmonary function test, the number of acute epi-sodes, intervals between attacks , SGRQ and MMRC were observed after drug administration in the two groups .Results The first second forced expiratory volume (FEV1), forced vital capacity (FVC), FEV1/FVC% and inspiratory capacity (IC) were significantly improved after being treated with tiotropium for 48 weeks (all P<0.05).Compared with the con-trol group, the values of FEV1, FVC, FEV1/FVC%and IC were significantly increased in the observation group (all P<0.05).The numbers of acute episodes was less , the intervals between attacks was longer and the life quality scores im-proved in both the two groups as compared with those before treatment (all P<0.05), and the observation group improved much better than the control group .Conclusion Tiotropium could improve pulmonary function , reduce the number of ex-acerbations and intervals , improve quality of life in patients with stable COPD with high safety , which is worthy of clinical promotion.
