广州民警血清尿酸水平与高血压和心电图缺血性 ST-T 改变的关系探讨

         

摘要

目的:探讨广州民警血清尿酸( UA)水平与高血压及心电图缺血性ST-T改变的关系。方法广州市海珠区公安民警2120例,按照血清UA水平分为4个组:UA<349μmol/L为低水平组,UA 349~408μmol/L为中等较低水平组,UA 409~472μmol/L为中等较高水平组,UA>472μmol/L为高水平组。比较各组血压、心电图缺血性ST-T改变及TG、CHOL、LDL-C、HDL-C、空腹血糖( FBG )、ALT、AST、UA、肌酐( CRE )水平。结果高血压335例,UA水平为(445.6±99.2)μmol/L;非高血压1785例,UA水平为(406.6±93.5)μmol/L;P<0.01。无心电图缺血性ST-T改变1938例,UA水平为(414.9±93.9)μmol/L;有缺血性ST-T改变182例,UA水平为(386.0±106.1)μmol/L;P<0.01。多因素非条件性Logistic回归分析发现,年龄、Hb、RBC、ALT、TG、UA、ST-T改变、B超脂肪肝表现可能是高血压病的危险因素;DBP、GLU、AST可能是心电图缺血性ST-T改变的危险因素。结论广州市海珠区公安民警血压水平及高血压病发病率随UA水平升高而升高,UA可能是高血压病独立危险因素。 UA在低水平及高水平可能导致心电图缺血性ST-T改变发生率升高;但当UA达到一定高水平后,心电图缺血性ST-T改变发生率可能有所回落。%Objective To discuss the relationships between uric acid ( UA) and hypertension or electrocardiogram is-chemic ST-T changes of Guangzhou policemen .Methods Totally 2 120 policemen of Haizhu District of Guangzhou were di-vided into four groups according to their UA levels as following: UA<349 μmol/L was the low level group , UA 349-408μmol/L was the medium relatively low group , UA 409-472μmol/L was the medium relatively high level group and UA >472μmol/L was the high level group .The blood pressure, the incidence of ECG ischemic ST-T changes, TG, CHOL, LDL-C, HDL-C, fast blood glucose (FBG), ALT, AST, UA, creatinine (CRE) levels between those groups were compared and ana-lyzed.Results There were 335 cases suffering hypertension , whose UA levels were (445.6 ±99.2) μmol/L; and there were 1 785 cases without hypertension , whose UA levels were (406.6 ±93.5)μmol/L (P<0.01).There were 1 938 cases without ischemic ECG ST-T changes, whose UA levels were (414.9 ±93.9) μmol/L;and there were 182 cases with ische-mic ECG ST-T changes, whose UA levels were (386.0 ±106.1) μmol/L;P<0.01.Multivariate unconditional logistic re-gression analysis suggested that age , Hb, RBC, ALT, TG, UA, ST-T changes and fatty liver of B ultrosound all might be the risk factors for hypertension;DBP, GLU and AST might be the risk factors for ischemic ECG ST-T changes.Conclusions Both of the blood pressure levels and prevalence of hypertension rise with the increase of UA levels .UA may be an independ-ent factor for hypertension .Either of the low levels or high levels of UA may increase the incidence of ECG ischemic ST -T changes.But when UA reaches a certain high level , the incidence of ECG ischemic ST-T changes may decline.
