2型糖尿病伴大血管病变患者血清高敏C反应蛋白水平变化及意义

         

摘要

Objective To investigate the level change of serum hypersensitive c-reactive protein ( hs-CRP) and its sig-nificance in type 2 diabetes mellitus (T2DM) patients with vascular lesions.Methods Selection of 45 patients with T2DM combined large vascular lesions (group A) and 45 cases of pure T2DM patients (group B) and 40 cases of healthy physical examination ( group C) , with transmission turbidimetric method to detect serum hs-CRP, using automatic biochemistry ana-lyzer determination of blood lipid , micro column method was used to measure blood sugar .The relationship among the ser-um hs-CRP and blood glucose , blood lipid in group A was performed by Pearson correlation analysis .Results The serum hs-CRP in group A, B, C was(4.12 ±2.15), (2.16 ±0.92), (0.62 ±0.34) ng/mL, glycosylated hemoglobin (HbA1c) was 11.09%±1.67%, 9.27%±1.21%, 4.17%±0.41%, fasting blood glucose (FPG) was (11.86 ± 3.91), (10.97 ±3.36), (4.58 ±0.76)mmol/L, postprandial 2 hours blood glucose (PPG) was (15.91 ±2.34), (13.79 ±3.29), (6.16 ±0.83)mmol/L.Comparison of serum hs-CRP among the three groups, P<0.01.Group A and group B compared with group C of HbA1c, FPG, PPG, P<0.01.The level of TC in group A, B, C was(5.84 ±0.56), (5.47 ±0.34), (3.31 ±0.25)mmol/L, TG was (2.27 ±0.27), (2.18 ±0.23), (1.21 ±0.19)mmol/L, LDL-C was (4.46 ±0.91), (4.34 ±0.67), (2.07 ±0.31)mmol/L, HDL-C was(1.17 ±0.37), (1.21 ±0.42), (1.51 ±0.62) mmol/L, the level of TC, TG, LDL-C, HDL-C in group A, B compared with group C, P<0.01.Correlation analysis re-sults showed that serum hs-CRP and HbA1c , FPG, PPG, TC, TG, LDL-C were positively correlated ( r were 0.368 5, 0.439 3, 0.384 7, 0.306 9, 0.343 1, 0.404 5, P<0.01), and negatively correlated with HDL-C (r=0.239 3, P<0.01).Conclusion The level of serum hs-CRP increase significantly in T2DM patients with vascular lesions .The hs-CRP may participate in the T2DM of the pathological process of vascular lesions .%目的:观察2型糖尿病(T2DM)伴大血管病变患者血清高敏C 反应蛋白(hs-CRP)水平变化,并探讨其意义。方法选取45例T2DM合并大血管病变患者(A组)、45例单纯T2DM患者(B组)及40例健康体检者(C组),采用透射比浊法检测血清hs-CRP,采用全自动生化仪测定血脂,采用微柱法测定血糖。对A组患者血清hs-CRP与血糖、血脂的关系采用Pearson法进行相关性分析。结果 A、B、C组血清hs-CRP分别为(4.12±2.15)、(2.16±0.92)、(0.62±0.34)ng/mL,糖化血红蛋白(HbA1c)分别为11.09%±1.67%、9.27%±1.21%、4.17%±0.41%,空腹血糖(FPG)分别为(11.86±3.91)、(10.97±3.36)、(4.58±0.76)mmol/L,餐后2小时血糖(PPG)分别为(15.91±2.34)、(13.79±3.29)、(6.16±0.83)mmol/L,三组间血清hs-CRP比较,P均<0.01;A、B组与C组HbA1c、FPG、PPG比较,P均<0.01。 A、B、C组TC分别为(5.84±0.56)、(5.47±0.34)、(3.31±0.25)mmol/L,TG分别为(2.27±0.27)、(2.18±0.23)、(1.21±0.19)mmol/L,LDL-C分别为(4.46±0.91)、(4.34±0.67)、(2.07±0.31) mmol/L,HDL-C分别为(1.17±0.37)、(1.21±0.42)、(1.51±0.62)mmol/L,A、B组TC、TG、LDL-C、HDL-C与C组比较,P均<0.01。相关分析结果显示,血清hs-CRP与HbA1c、FPG、PPG、TC、TG、LDL-C均呈正相关( r分别为0.3685、0.4393、0.3847、0.3069、0.3431、0.4045,P均<0.01),与HDL-C呈负相关(r=-0.2393,P<0.01)。结论 T2DM伴大血管病变患者血清hs-CRP水平明显升高;hs-CRP可能参与了T2DM血管病变的病理过程。
