强化降脂对急性脑梗死患者颈动脉粥样硬化斑块稳定性及血清基质金属蛋白酶9水平的影响研究

摘要

Objective To observe the influence of intensive lipid - lowering on carotid atherosclerotic plaques stability and serum matrix metalloproteinases 9(MMP - 9)in patients with acute cerebral ischemia. Methods A total of 60 patients with acute cerebral infarction admitted to our hospital from October 2012 to October 2013 were selected,and then they were di-vided into control group(n = 30) and observation group( n = 30) according to random number table. Based on conventional therapy,control group given rosuvastatin at dose of 10 mg/ d,observation group given rosuvastatin at dose of 20 mg/ d. Blood liq-uid,serum MMP - 9 level of the two groups were detected before treatment and 6 months after treatment,and carotid atheroscle-rotic plaques stability observed. Results The rate of reaching standard for decreasing blood liquid of observation group was 73. 3% ,was higher than that of control group of 33. 3% (P < 0. 05). There was no significant differences of serum MMP - 9 level and detectable rate of unstable plaques between the two groups before treatment(P > 0. 05);serum MMP - 9 level and de-tectable rate of unstable plaques were lower after treatment than before(P < 0. 05),and above index after treatment of observa-tion group were lower than those of control group(P < 0. 05). Conclusion Intensive lipid - lowering can improve the rate of reaching standard for decreasing blood liquid,decrease serum MMP - 9 level and enhance carotid atherosclerotic plaques stability in patients with acute cerebral ischemia.%目的：探讨强化降脂对急性脑梗死患者颈动脉粥样硬化斑块稳定性及血清基质金属蛋白酶9（MMP -9）水平的影响。方法选择我院2012年10月-2013年10月住院急性脑梗死患者60例，采用随机数字表法分为对照组和观察组，每组30例。在常规治疗基础上，对照组患者给予瑞舒伐他汀10 mg/ d，观察组患者给予瑞舒伐他汀20 mg/ d。检测两组患者治疗前、治疗6个月后血脂、血清 MMP -9水平，观察其颈动脉粥样硬化斑块情况。结果观察组患者血脂达标率为73.3%，高于对照组的33.3%（P <0.05）。两组患者治疗前血清 MMP -9水平、不稳定性斑块检出率比较，差异均无统计学意义（P >0.05），两组患者治疗后血清 MMP -9水平、不稳定性斑块检出率均较治疗前降低（P <0.05），且观察组患者治疗后血清 MMP -9水平、不稳定性斑块检出率均低于对照组（P <0.05）。结论强化降脂有利于提高急性脑梗死患者血脂达标率、更好地降低急性脑梗死患者血清 MMP -9水平，增加颈动脉粥样硬化斑块稳定性。
