肾功能损害标志物测定在慢性肾脏病诊断中的临床价值

         

摘要

Objective To investigate the levels of renal injury biomarkers in patients with chronic kidney disease(CKD) and evaluate their clinical significances in diagnosis of CKD. Methods A total of 66 subjects(37 patients with CKD and 29 healthy individuals) were involved in this study. Serum blood urea nitrogen (SBUN) was determined by Glutamate dehydrogenase method; serum creatinine(SCr) and urinary creatinine(UCr) were detected by sarcosine oxidase method; serum uric acid(SUA) was measured by uncase colorimetry; serum cystatin C (Cys C) and urinary microalbumin(UmAlb)were analyzed by immunological transmission turbidimetry; urinary protein (U-PRO)was measured by Coomassies Brilliant Blue(CBB) assay. The UmAlb and U-PRO levels were expressed in units of mg/ mmolUCr. Results The results of independent samples t test indicated that significant differences were found in SBUN. SCr, SUA. Cys C. UmAlb and U-PR(P<0. 05) between patient group and healthy control group. The evaluation of diagnostic effects showed that the areas under the curve at ROC plot for SBUN, SCr, SUA, Cys C, UmAlb and U-PRO were 0. 907, 0. 912, 0. 742. 0. 982, 0. 984 and 0. 991, respectively. Conclusions U-PRO, UmAlb and Cys C are ideal biomarkers. SCr and SBUN come next, SUA is the weakest when the above biomarkers are applied to evaluate the renal injury and its severity of the patients with CKD.%目的 研究慢性肾脏病(CKD)患者肾功能损害标志物水平的变化,评价其在CKD诊断中的意义.方法 研究对象共66例,其中病例组(CKD患者)37例,对照组29例.采用谷氨酸脱氧酶法检测血清尿素氮(SBUN)浓度,肌氨酸氧化酶法测定血清肌酐(SCr)和尿肌酐(UCr)含量,尿酸酶比色法测定血清尿酸(SUA)含量.免疫透射比浊法分析受试者血清胱抑素C(Cys C)和尿微量白蛋白(UmAlb)水平,考马斯亮蓝(CBB)法测定尿蛋白(U-PRO)含量.UmAlb、U-PRO水平均以mg/mmolUCr为单位表示.结果 独立样本t检验结果表明,病例组与对照组比较,SBUN、SCr、SUA、CysC、UmAlb和U-PRO差异有统计学意义(P.<0.05).诊断效果评价表明,SBUN、SCr、SUA、Cys C、UmAlb和U- PRO在ROC曲线下的面积分别为0.907、0.912、0.742、0.982、0.984和0.991.结论 在评价CKD患者肾功能损害及其程度时,以U-PRO、UmAlb和CysC为最佳,SCr、SBUN次之,SUA居后.
