乳腺化生性癌8例临床病理分析

摘要

Objective:To study the clinical and pathological features,differential diagnosis and prognosis of breast metaplastic carcinoma.Methods:To study the clinical manifestations,histopathological and immunohistochemical fea-tures of 8 cases of breast metaplastic carcinoma.Results:There were 2 cases of squamous cell carcinoma,6 cases of carcinosarcoma.Including 2 patients with cartilaginous metaplasia.Immunohistochemical expressed p63,CKH in squa-mous cell carcinoma,sarcomatoid carcinoma epithelial components of the expression of Ckpan,mesenchymal compo-nent expression of Vimentin,S-100 expression and cartilage differentiation.Lymph node metastasis in 2 cases.Follow-up for 6,3 cases with recurrence,1 case alive,2 cases dead.Conclusion:Metaplastic carcinoma of the breast is very rare,diagnosis including breast infiltrative ductal carcinoma,primary breast sarcoma and malignant phyllodes tumors.Ckpan,Vimentin,S-100,p63 and CKH are useful marker.%目的：探讨乳腺化生性癌的临床病理特征、鉴别诊断及预后。方法：对我院8例乳腺化生性癌进行临床表现、组织病理学和免疫表型观察。结果：2例为鳞状细胞癌，6例为癌肉瘤，其中2例伴软骨化生。免疫组化检测鳞状细胞癌表达p63、CKH，癌肉瘤上皮成分表达Ckpan，间叶成分表达Vimentin，伴软骨分化者表达S-100。2例淋巴结转移，完整随访6例，3例复发，1例存活，2例死亡。结论：原发于乳腺的化生性癌较罕见，诊断需与乳腺浸润性导管癌3级、乳腺原发性肉瘤、恶性叶状肿瘤等相鉴别。免疫组化Ckpan、Vimentin、S-100、p63、CKH等是一组有用的标记物。
