Bmi -1在宫颈癌中的表达及与 HR - HPV 感染的相关性研究

摘要

目的：研究 Bmi -1蛋白、高危型人乳头瘤病毒（HR - HPV）在正常宫颈、宫颈癌、癌前病变组织中的表达及与宫颈癌临床病理特征的关系，探讨两者的临床意义及相关性。方法：采用免疫组化（SP）方法检测16例正常宫颈、45例宫颈上皮内瘤变（CIN）、41例宫颈癌中 Bmi -1、HR - HPV 的表达，分析其临床意义及相互关系。结果：Bmi -1在宫颈正常组、CINⅠ组、CINⅡ-Ⅲ组及宫颈癌组中的阳性率呈递增趋势，分别为：6.3%、9.1%、44.1%、73.2%。HR - HPV 在宫颈正常组、CINⅠ组、CINⅡ-Ⅲ组及宫颈癌组中的阳性率呈递增趋势，分别为：12.5%、27.3%、61.8%、80.5%。Bmi -1在宫颈癌组织中的表达率与临床分期、肿瘤分级及淋巴结有无转移相关，而与病理分型及年龄无关。HR - HPV 的阳性表达率与宫颈癌临床病理参数及年龄无关。Bmi -1与 HR - HPV 在宫颈癌中的表达呈正相关。结论：Bmi -1在宫颈癌组织中呈过表达且与临床分期、肿瘤分级及淋巴结转移有一定的相关性，提示它可能参与了宫颈癌的发生、发展，在一定条件下可以作为宫颈癌的一个预后指标。在宫颈癌中 Bmi -1与 HR - HPV 感染相关，提示两者可能在导致宫颈癌发生过程中起协同作用。%Objective:To investigate the expression of Bmi - 1 protein,HR - HPV in normal cervix,cervical carci-noma,precancerous lesions and its relationship with clinicopathological features. Methods:Immunohistochemistry (SP)was used to detect the expression of Bmi - 1,HR - HPV methd in 16 cases of normal cervix,45 cases of cervi-cal intraepithelial neoplasia(CIN),41 cases of cervical carcinoma. Results:The expression rate of Bmi - 1 in control group,CINⅠ group,CINⅡ - Ⅲ group and cervical carcinoma group had an significantly increasing trend:6. 3% , 9. 1% ,44. 1% ,73. 2% . The expression rate of HR - HPV in these groups also showed an increasing trend:12. 5% , 27. 3% ,61. 8% ,80. 5% . It was suggested that Bmi - 1 in cervical carcinoma was correlated with the clinical stage, tumor grading and lymph node metastasis,regardless of the pathological type and age. The expression rate of HR -HPV in cervical carcinoma was regardless of its clinicopathologic significance and age. The expression of Bmi - 1 and HR - HPV showed a positive correlation in cervical carcinoma. Conclusion:Bmi - 1 is overexpressed in cervical car-cinoma and it correlated with the clinical stage,tumor grading and lymph node metastasis,which suggests that Bmi -1 might participate in the development and progression of cervical carcinoma,it have clinical potential of a promising prognostic predictor of cervical carcinoma. The correlation between Bmi - 1 and HR - HPV in cervical carcinoma shows that probably they could play an role in the occurrence of cervical carcinoma.
