Objective:To investigate the pathological characteristics and prognosis of young female patients with gastric cancer. Methods:Clinical data of 48 young female patients(≤40years)with gastric cancer from May,2006 to December,2012 in Nanjing Drum Tower Hospital were retrospectively collected and analyzed,along with the postoper-ative pathological data and survival time of 149 old female patients treated at the same time were served as compari-son. Results:Young female patients with gastric cancer accounted for 8. 4% of the all included female patients,in which the vast majority of patients received radical resection. In the cohort of young female patients,postoperative pathological examination showed that most lesion were located in gastric body,antrum and small curvature,and patho-logical types were commonly poorly differentiated adenocarcinoma and signet ring cell carcinoma. Moreover,Lauren′s type were mostly diffuse type in female patients.χ2 test showed a significant difference of the pathological pattern be-tween young and old female patients(p<0. 01). Furthermore,young female patients had a higher rate of vessel inva-sion and lymph node metastasis than old female patients. In life test,there were 15 patients with the stage III-IV gas-tric carcinoma in each cohort. In comparison,the median survival time of patients in young group and old group was 11. 8 months versus 21. 5 months(p=0. 035). Conclusion:Young female patients with gastric carcinoma have higher degree of malignancy and worse outcome,therefore this cohort should be early diagnosed and treated.%目的：探讨青年女性胃癌患者的术后病理特点及预后分析。方法：回顾性分析2006年5月至2012年12月我院收治的48例青年女性（小于或等于40岁）胃癌患者的临床资料，与同期60-69岁老年女性胃癌患者共149例的术后病理资料及生存时间进行比较。结果：青年女性胃癌患者占全部女性胃癌患者的8.4%，绝大多数行根治术，术后病理示肿瘤多侵犯胃体、胃窦及小弯侧，病理类型上以低分化腺癌及印戒细胞癌为主，Lauren′s分型中弥漫型居多，相比于老年女性胃癌患者组有明显统计学差异（ p<0.01）。另其脉管受侵、淋巴结转移率也较高。随访两组中III-IV期患者各15例，青年女性胃癌组中位生存时间11.8个月，相比于老年组中位生存期21.5个月，生存时间明显缩短（ p=0.035）。结论：相比于老年女性，青年女性胃癌恶性程度高、预后差。
