恶性血液病继发第二肿瘤研究进展

摘要

The incidence of hematologic malignancies has increased in recent years.Survival of these patients have improved dramatically with the deepening understanding of diagnosis and treatment and the introduction of new agents.However,the late related toxicity especially the second cancer also increases gradually and affects the patient's quality of life seriously.Therefore,the occurrence of the second primary malignancy has become a new clinical challenge of treatment process.Etiologic factors for second malignancy remain elusive.Here we review current understanding of chemotherapy related factors, the diseases and patients specific risk factors to investigate the clinical characteristics, therapeutic effect, prognosis and strategies.%近年来恶性血液病发病率有逐年增高的趋势,随着对其诊治研究的深入,治疗方案的不断改进,长期生存患者的比例不断提高,但是其相关的迟发毒性尤其是第二肿瘤的发生也逐渐增多并严重影响着患者的生存质量.因此,第二肿瘤的发生已成为恶性血液病治疗过程中一个新的临床挑战.关于血液系统恶性疾病继发第二肿瘤的机制尚有待明确,本文就其治疗相关因素、疾病本身特点及宿主本身因素的最新研究进展作一综述,探讨恶性血液病继发第二肿瘤的临床特点、疗效、预后及治疗策略.
