目的:通过检测血清中高尔基体蛋白73(GP73)、EB病毒(EBV)-DNA和血清病毒衣壳蛋白抗原IgA(VCA-IgA)水平,探讨它们在鼻咽癌诊断中的价值.方法:收集鼻咽癌患者45例(其中未转移患者21例,转移患者24例),EBV抗体阳性人群49例(其中EBV抗体阳性持续在1年至5年以内22例,EBV抗体阳性持续在5年以上27例),正常健康体检人群20例,分别用ELISA法检测血清GP73和VCA-IgA,实时荧光定量PCR法检测EBV-DNA水平,并进行数据分析.结果:鼻咽癌组的EBV-DNA、GP73含量和阳性率明显高于其它组(P<0.05);鼻咽癌、EB-IgA阳性组的GP73含量明显高于健康对照组(P<0.05);EB-IgA阳性在体内持续时间越长,GP73含量有逐渐升高的趋势.鼻咽癌转移组GP73阳性率明显高于未转移组(P<0.05),鼻咽癌转移组与未转移组比较,EBV-DNA阳性率无统计学意义(P>0.05).结论:测定血液中EBV-DNA、GP73蛋白检测有助于鼻咽癌早期诊断,GP73蛋白还有助于判断鼻咽癌是否转移.%Objective:The value of serum GP73, EBV-DNA and VCA-IgA in the diagnosis of nasopharyngeal carcinoma were studied.Methods:45 cases of nasopharyngeal carcinoma were collected(nonmetastasis in 21 cases, metastasis in 24 cases),EB virus antibody positive population in 49 cases(including EB virus antibody positive in 1 year to 5 years in 22 cases,EB virus antibody positive for more than 5 years in 27 cases),20 cases of normal healthy people.Detection of serum GP73 and VCA-IgA was did by using ELISA method, EBV-DNA using Real-time PCR method,and data analysis.Results:EBV-DNA and GP73 in nasopharyngeal carcinoma and the positive rate were significantly higher than that of other groups(P < 0.05).GP73 in nasopharyngeal carcinoma, EB-IgA positive group was significantly higher than healthy control group(P < 0.05).With EB-IgA positive in vivo duration,the content of GP73 had increased trend.The positive rate of GP73 in nasopharyngeal carcinoma metastasis group was significantly higher than that in the non metastasis group(P < 0.05), and the positive rate of EBV-DNA was not statistically significant in nasopharyngeal carcinoma(NPC) metastasis group and non metastasis group(P > 0.05).Conclusion:Determination of EBV-DNA and GP73 protein in the blood is helpful to the early diagnosis of nasopharyngeal carcinoma, and GP73 protein may be helpful in the diagnosis of nasopharyngeal carcinoma metastasis.
