超声刀在食管癌Sweet手术中的应用

         

摘要

目的:探讨超声刀在食管癌左胸一切口根治术(Sweet术)中的应用价值.方法:2011年1月至2012年6月使用超声刀切除食管中下段鳞癌28例,对照组20例,比较两组在手术时间、术中出血量、淋巴结清扫数目及术后72h引流量的差异.结果:超声刀组和对照组手术时间分别为(166±24) min和(206±32) min;术中出血量分别为(283±59)ml和(310±47)ml;术后72h引流量分别为(989±214) ml和(1 203±403)ml;术中淋巴结清扫数分别为(19.5±7)枚和(16.5±6)枚;两组比较总手术时间和术后72h引流量差异有统计学意义(P<0.05);术中出血量及淋巴结清扫数目两组比较无统计学意义.结论:超声刀可缩短食管Sweet手术时间,减少术后的胸腔引流,有助于淋巴结的清扫.%Objective:To investigate the safety and feasibility of ultrasonic scalpel in the Sweet operation for esophageal carcinoma.Methods:Totally 48 patients with middle and lower parts esophageal carcinoma were divided into two groups:Ultrasonic scalpel group (28 cases) and control group (20 cases).The operation time,bleeding volume,the number of lymph nodes and the volume of postoperative thoracic drainage were compared.Results:The average operation time and postoperative 72h quantity of thoracic drainage in the study group were significantly lower than those in the control group (P < 0.05).There was no significant difference between the two groups in the volume of bleeding and the number of lymph nodes (P > 0.05).Conclusion:Ultrasonic scalpel can be safely and reliably used to the Sweet operation for esophageal carcinoma with less operation time and less postoperative thoracic drainage.
