p73基因G4C14-to-A4T14多态性与中国西北人群胃癌高发风险的相关性

         

摘要

目的 评价p73基因G4C14-to-A4T14双核苷酸多态性与中国西北人群胃癌高发风险以及胃癌不同病理亚型的相关性.方法 采用两双相对引物的聚合酶链式反应法,对p73基因G4C14-to-A4T14多态性基因分型.结果 共检测了385例胃癌患者和412名健康对照者.胃癌组中有弥漫型胃癌305例(79.22%),肠型胃癌80例(20.78%).对照组p73 AT/AT、AT/GC及GC/GC基因型的频率分别为28.15%、47.09%和24.76%,胃癌组分别为21.98%、45.04%和32.98%.以AT/AT作为指示物,胃癌组(主要是弥漫型胃癌病例)的GC/GC纯合子基因型频率高于对照组,优势比分别为1.71(95%CI,1.16~2.51)和1.87(1.24~2.81);胃癌组(主要是肠型胃癌)病例的AT/GC杂合子频率也高于对照组,优势比分别为1.23(95%CI,0.86~1.74)和1.40(95%CI,0.70~2.54).结论 p73基因G4AGC/GC纯合子基因型与我国西北人群胃癌,特别是弥漫型胃癌的高发风险相关;AT/GC基因型携带者以患肠型胃癌的风险更高.在胃癌发病率居中国首位的西北人群中,弥漫型胃癌占大多数.%Objective To evaluate the genetic association of p73 gene G4C14-to-A4T14 double nucleotide polymorphism with increased gastric cancer (GC) risk and different histological sub- types of GC in northwestern Chinese population. Methods The genotyping of polymorphisms of p73 gene G4C14-to-A4T14 was conducted with PCR-CTPP. Results A total of 385 GC pa- tients and 412 healthy controls were investigated. There were 305 (79.22%) diffuse-type cases and 80 (20.78%) intestinal-type cases in GC group. The frequencies of p73 AT/AT,AT/GC and GC/GC were 28.15%,47.09% and 24.76% in the controls,and 21.98%,45.04% and 32.98% in GC cases respectively. Using the AT/AT as the referent,the GC/GC homozygous genotype fre- quency was higher in GC cases,mainly in diffuse-type GC cases,than that in the controls with OR=1.71 (95% CI,1.16~2.51) and 1.87 (1.24~2.81) respectively; AT/GC heterozygote frequency was also higher in GC group,mainly in intestinal type GC cases,than that in the controls with OR=1.23(95% CI,0.86~1.74) and 1.40(95% CI,0.70~2.54). Conclusion p73 G4A GC/GC homozygote is associated with increased GC risk,especially with diffuse-type GC in northwestern Chinese population. Carriers of AT/GC genotype have slightly increased risk of GC,mainly of intestinal-type GC. Diffuse-type GC is dominant in northwestern Chinese population with the highest incidence of GC in China.
