肥胖患者血清Apelin水平与血糖、血脂和胰岛素抵抗的相关性分析

         

摘要

Objective To observe the correlation of the serum levels of Apelin to blood glucose, lipids and insulin resistance in obese patients. Methods 64 obese patients who were treated in our hospital from October 2016 to October 2017 were selected as the observation group, and 64 healthy people who underwent physical examination during the same period were enrolled as the control group. The serum Apelin levels were measured. In the meantime, those in the observation group underwent ora glucose tolerance test, and fasting blood glucose, 2 h postprandial blood glucose (2 h PG), triglyceride and fasting insulin were measured. Homeostasis model assessment-insulin resistance index (HOMA-IR) was calculated. The correlation between serum Apelin levels and blood glucose, lipids and insulin resistance in obese patients were analyzed by Pearson correlation analysis. Results The serum levels of Apelin in the observation group and the control group were (446.54±45.32) ng/L and (378.42±38.25) ng/L respectively, and the difference was statistically significant (t=9.189, P<0.001). The observation group included 28 cases of normal glucose tolerance (NGT), 24 cases of impaired glucose tolerance (IGT) and 12 cases of type 2 diabetes (T2 DM). The serum Apelin levels of the IGT and T2 DM groups were higher than those of the NGT group, suggesting significant differences (P<0.05). The serum Apelin levels of the T2 DM group were significantly higher than those of the IGT group (P<0.05). Pearson correlation analysis showed that the serum Apelin levels in obese patients were positively correlated with 2 hPG, triglycerides and the HOMA-IR (r=0.842, P<0.001; r=0.938, P=0.000; r=0.916, P<0.001). Conclusion The serum levels of Apelin in obese patients are positively correlated with blood glucose, lipids and insulin resistance, which indicates that Apelin plays a certain role in the occurrence and development of obesity.%目的 探讨肥胖患者血清Apelin水平与血糖、血脂和胰岛素抵抗的相关性.方法 选取2016年10月—2017年10月我院收治的肥胖患者64例作为观察组,选择同期健康体检者64例作为对照组.检测血清Apelin水平,同时观察组行口服葡萄糖耐量试验,并检测空腹血糖、餐后2 h血糖、甘油三酯和空腹胰岛素,计算胰岛素抵抗指数;采用Pearson相关分析评估肥胖患者血清Apelin水平与血糖、血脂和胰岛素抵抗的相关性.结果 观察组、对照组血清Apelin水平分别为(446.54±45.32)ng/L、(378.42±38.25)ng/L,比较差异有统计学意义(t=9.189,P<0.001).观察组糖耐量正常(normal glucose tolerance,NGT)28例、糖耐量减低(impaired glucose tolerance,IGT)24例和2型糖尿病(type 2 diabetes,T2DM)12例.与NGT组比较,IGT组、T2DM组血清Apelin水平升高,差异有统计学意义(P<0.05);与IGT组比较,T2DM组血清Apelin水平显著升高,差异有统计学意义(P<0.05).Pearson相关分析显示,肥胖患者血清Apelin水平与餐后2 h血糖、甘油三酯和胰岛素抵抗指数呈正相关(r=0.842,P<0.001;r=0.938,P=0.000;r=0.916,P<0.001).结论 肥胖患者血清Apelin水平与血糖、血脂、胰岛素抵抗均呈正相关,表明Apelin在肥胖患者的发生发展中具有一定的作用.
