目的 探讨抗毒蕈碱药物联合阿米替林和中药治疗女性膀胱疼痛综合征的临床效果.方法 患者记录排尿日记,口服抗毒蕈碱药物、阿米替林片和中药制剂,疗程1个月,观察24h排尿次数、夜尿次数、每次排尿量、膀胱区疼痛评分、生活质量评分.治疗后1周、2周和4周进行疗效评估.结果 治疗前与治疗后各项观察指标比较差异均有统计学意义(P＜0.01),治疗1周与治疗2周、4周比较,各项观察指标差异均有统计学意义(P＜0.05),治疗2周与治疗4周比较差异亦有统计学意义(P＜0.05).疗程结束,31例症状完全消失,11例仍有轻微尿频,7例有轻微膀胱区憋胀,5例有尿道口轻微疼痛,但均能耐受.结论 抗毒蕈碱药物联合阿米替林和中药可以有效治疗女性膀胱疼痛综合征,提高生活质量.%Objective To explore the effect of antimuscarinic drug combined with amitriptyline and Chinese herbal drugs in treatment of painful bladder syndrome in women. Methods The patients were advised to keep a micturition diary and to take antimuscarinic drug orally, amitriptyline and Chinese herbal drugs for 1 month. Urination frequency in 24 hours, nocturia frequency, each urinary output, the score of pain in bladder area and the score of life quality were observed. Curative effect evaluation was performed on 1st, 2nd and 4th week after treatment. Results The differences of observed indexes before and after treatment were statistically significant (P <0.01), the differences in the 1st, 2nd and 4th week after treatment were also statistically significant (P <0.05), and the differences in the 1st and 2nd week after treatment was statistically significant (P<0.05). The 31 patients were cured after 1 month of treatment, 11 patients with mild urinary frequency, 7 patients with mild bladder regional distension and 5 patients with mild urethral orifice pain. But the pains were tolerable. Conclusion Antimuscarinic drug combined with amitriptyline and Chinese herbal drugs may improve the symptoms and quality of life of the patients with painful bladder syndrome.
