目的:探讨根据应力方向分析复合颅面骨骨折的方法.方法:对48例颅面骨病例行多层螺旋CT容积扫描和三维重建,沿撞击力方向、角度分析和三维重建图像分析颅面骨各部位骨折特点.结果:当着力点位于颅面骨,应力沿撞击点向周围传导,出现应力型骨折比例77.3%(37/48)、关节脱位33%和副鼻窦损伤35.4%.在骨结构的横截面积陡然减小的区域出现应力集中,出现骨折50%.结论:三维重建和机械应力分析综合法能够全面分析和提高认识颅面骨骨折特点.%Objective To detect a way to analyze complex cranium-face bones fracture according to stress direction. Methods Volume scan data and three-dimensional reconstruction of cranium-face bone fraction were obtained in forty-eight cases. Fracture features were analyzed by using stress mechanics analysis change along with stress direction, angle and 3D reconstruction. Results While mechanical stress put on cranium-face bone, stress spread from stress pot to surrounding structure .resulting in 77.3% stress mechanical fracture, joint dislocation 33% and paranasal sinus fraction 35.4% . Conclusion Mechanical stress and 3D reconstruction analysis can improve the understanding of craniofacial fracture feature.
