弥散张量成像的纤维束追踪重建面神经可行性研究

         

摘要

目的初步探讨听神经瘤患者术前应用基于弥散张量成像的纤维束追踪技术进行面神经重建的可行性.方法使用3.0T磁共振采集2例入组患者的循环相位稳态采集快速成像和弥散张量成像序列的数据,3D Slicer软件进行基于弥散张量成像的纤维束追踪,对面神经进行三维重建,并在手术中验证追踪重建出的面神经位置及走行的准确性.结果2例患者利用基于弥散张量成像的纤维束追踪技术均成功重建出经过设定的内听道及面神经入脑干区两个ROI区的面神经,可直观地观察到面神经与肿瘤的位置关系和走行,并在术中与面神经的实际位置相对比是一致的.结论应用基于弥散张量成像的纤维束追踪技术重建面神经存在一定的可行性,可帮助术者在术前直观的观察到面神经与肿瘤的位置及走行.%Objective To study the feasibility of facial nerve 3-D reconstruction through diffusion tensor imag⁃ing-based fiber tracking in patients with acoustic neuroma. Methods Magnetic resonance images collected on a 3-T scan⁃ner, including Fast Imaging Employing Steady State Acquisition with Cycled Phases (FIESTA-C) and diffusion tensor imag⁃ing (DTI), as well as anatomic images, were processed based on DTI fiber tracking using the 3D Slicer software in 2 patients with acoustic neuromas. The accuracy of tumor and facial nerve 3-D reconstruction was verified during surgery. Results In both cases, using DTI-based fiber tracking, 3-D reconstruction of the facial nerve through two ROIs including the internal auditory canal and the nerve entry zone was successful and demonstrated the relations between the facial nerve and tumor, which were later confirmed by intraoperative findings. Conclusion Facial nerve 3-D reconstruction through diffusion tensor imaging-based fiber tracking is feasible, and can be helpful in obtaining a visual observation preoperatively of the position of the facial nerve in relation to acoustic neuroma.
