骨科椎管内麻醉患者术后首次饮食时间的研究

         

摘要

Objective To investigate the proper time of postoperative oral feeding in patients undergoing orthopedic surgery under spinal anesthesia. Methods Five hundred and twenty patients undergoing orthopedic surgery under spinal anesthesia were recruited from three hospitals in Zibo area of Shandong Province. The patients were divided into the experimental group and the control group. The patients in the experimental group were allowed to take food once they had needs to eat and recovered to normal consciousness without stomach discomfort. The patients in the control group were not allowed to take food until 6 hours after operation. The symptoms such as thirst, hunger, nausea, vomiting, abdominal distention at five hours after operation and two hours after eating were compared between the two groups. Results There were significant differences in the symptoms of thirst and hunger between the two groups at five hours after operation (P<0.01). There was no significant difference in any symptoms between the two groups at two hours after eating. Conclusions Early oral feeding after operation is feasible according to the individualized demands.%目的 探讨骨科椎管内麻醉患者术后首次饮食的适宜时间.方法 在山东省淄博地区3家医院骨科选择520例骨科椎管内麻醉患者作为研究对象,以病区为单位分为观察组和对照组,每组260例.观察组术后有饮食需求时,如果意识清楚、无胃部不适即可先饮水少量,再进食;对照组则按常规术毕6h后才可进饮食.分别于术后5h和进食后2h评估两组患者口渴、饥饿感、恶心、呕吐、腹胀等情况,并进行比较.结果 术后5h对照组自觉有口渴、饥饿感的人数高于观察组(P<0.01).两组进食后2h各项观察指标差异无统计学意义(P>0.05).结论 骨科椎管内麻醉患者术后6h进饮食不应是惟一标准,评估患者的主观感受和客观体征后,根据患者需求术后提早开始饮食是可行的.
