This paper summarizes the follow-up services for nine patients receiving bilateral ureterostomy after discharge.During follow-up,it was found that most of the patients couldn't adapt to the stoma and suffered from anxiety and self-abasement,seven patients couldn't change ostomy bags by themselves,one patient used the self-made ostomy bag,two patients used the old-style ostomy bags,and the other six patients used pasted two pieces ostomy bags.One patient had urate crystal,one patient's bilateral stomas had rebounded,two patients had dermatitis and one of them had urinary tract infection.Aimed at the problems,targeted nursing interventions were provided,including providing follow-up services to ensure the continuity of nursing care,correcting the patients' attitudes toward the stoma to prevent self-image disorders,teaching the patients about the stoma care skills to improve their self-care agency,guiding the patients to choose proper stoma care materials and observe complications to improve their self-confidence and reduce the incidence of complications.It is suggested that the follow-up services can improve the patients' quality of life and promote them to return to the society in a better way.%总结9例双侧输尿管皮肤造口患者出院后随访及护理干预.随访后发现本组对双侧输尿管造口不适应,普遍存在焦虑自卑的心理;7例不能自己更换造口袋;1例使用自制简易式造口袋,2例使用旧式一次性造口袋,6例使用质量较好的两件式粘贴型造口袋.1例出现尿酸盐结晶,1例出现双侧造口回缩,2例出现造口周围皮炎,其中1例出现尿路感染.通过建立随访机制,确保护理干预的延续性;提高患者造口认知水平,消除自我形象紊乱;传授造口护理技巧,提升患者的自理能力;正确选择造口护理用品,指导并发症的观察和处理等,提升了患者的自我护理能力,增强了自信心,减少了并发症的发生.认为出院后的随访及护理干预,能提高造口患者的生活质量,促进患者形成良好的心态,更好地回归社会.
