射血分数正常的老年心力衰竭患者的临床观察

摘要

目的 探讨左室射血分数(LVEF)正常的老年心力衰竭患者的临床特点.方法 住院老年心力衰竭患者56例,LVEF正常心力衰竭组和LVEF降低心力衰竭组各28例,22例健康体检者为对照组,比较各组在体质量指数(BMI)、收缩压、舒张压、血浆N末端B型利钠肽(BNP)浓度的差异.结果 LVEF正常心力衰竭组和LVEF降低心力衰竭组美国纽约心脏病学会(NYHA)心功能分级无明显差异(P＞0.05).而BNP水平在LVEF正常心力衰竭组与对照组及LVEF降低心力衰竭组之间差异有统计学意义(P＜0.05).结论 多数有心力衰竭症状而LVEF正常者血浆BNP和血压均高.%Objective To discuss the clinical features of heart failure with normal ejection fraction in aged patients. Methods The data of 56 old patients with heart failure who admitted to our hospital were collected. Heart failure with preserved LVEF was diagnosed in 28 patients and heart failure with reduced LVEF was diagnosed in another 28 patients. 22 age-matched healthy subjects served as the control group. BMI,SBP,DBP, BNP were compared among the 3 groups. Results There was no difference in terms of NYHA classification between patients with preserved LVEF and reduced LVEF(P >0. 05). The plasma BNP was more significant in heart failure patients with reduced LVEF than those with preserved LVEF( P < 0. 05 ). Conclusion The full understanding of the clinical heart failure with normal left ventricular ejection fraction will improve the diagnostic accuracy.
